eCW Medical History Form

If you are an eCW customer using NexHealth Forms, you have access to the integrated Medical History Form, which automatically syncs patients’ Allergies, Conditions and Medications to the upcoming Progress Note in eCW.

Here’s how it works:

  1. The NexHealth Medical History Form is already included in your account. It features search-and-select free-text questions for patients’ Allergies, Conditions and Medications.
  2. After you send the Medical History Form, the patient types in their corresponding health history, which automatically pulls matching options from eCW.
  3. The patient’s Allergies, Conditions and Medications are automatically synced to the Progress Note field in the patient file in eCW. The PDF forms are synced to Patient Docs.

Q: Can I add more questions to the Medical History Form?
