How do I retrieve an expired Form?

Forms automatically expire to keep patient data secure and ensure PHI cannot be accessed beyond what is actually necessary for the patient’s visit. 

If a Form or Form Request has expired, you can retrieve it to allow the patients to reopen that form and make changes. 

To retrieve an expired Form:

  1. Navigate to the Forms tab and select the Expired filter. 
  2. Find the request you need to retrieve. 
  1. Click the  on the Actions option and select Move to active. 
  2. The request can then be found under the Active filter and the patient will be able to access the Form through the original link.
