NexHealth Support

NexHealth provides a collection of curated form templates for your use, with a customizable form builder. If, however, you find you need to add a new form, or get help making edits to an existing one, our team is here to help!

1. To submit your own form to be digitized, or to locate the available templated forms, navigate to Forms --> Manage Forms.

2. Click on New Form

3. Choose Digitize to upload a PDF of the form you would like our team to convert and add to your account. Digitized forms will typically be available within 72 business hours of the request.

4. Select Build to enter the Form Builder and select from our templated forms, which you can use as-is or edit.
