The Online Booking Form is the last step a patient encounters in the Online Booking flow. Here, they enter their demographic information so we can match the appointment with an existing profile in your health record system, or create a new patient.
To customize your Online Booking Form:
- Visit the Online Booking Form tab in Setup.
- Fields with the gray background are required to match patients to their profile. The blue dot beside a field indicates that patients will be required to submit that field to book.
- Optionally, you can also select Customize Form at the bottom of the page to add additional fields.
3a. Click Add Field to expand the list of options, including text boxes to provide additional informations, checkboxes, drop downs, date selections, and even requiring Payment information.
